Community Connections Cupar
Community Connections Cupar provides adults over the age of 16 with the opportunity to receive support from trained members of staff to develop an action plan to help with such areas as welfare benefits, job clubs and access to arranged local activities in the local area. This service will run on a Tuesday, there is no need to see a GP simply telephone 01334 651201 and ask for an appointment at reception.

The majority of people with diabetes have Type 2 Diabetes. If you’re a man over 40, your risk of developing Type 2 Diabetes increases with older age and excess weight. We are here to help.

New Patient Register
To make things easier for you to register, download a Registration Form and Questionnaire Form. Complete these at your leisure, print them off and bring them into the practice. It is practice policy to ask for the completed forms to be accompanied by photographic ID and proof of your address. Click here for more information on the Registration process.
