Registration Process

If you wish to register permanently with the practice you should present at the surgery, where you will be given 2 forms to complete.

You can, if you wish, print these forms off in advance and fill them in before you come: Health Questionnaire.Registration Form Oct 2023

The practice requests that along with your completed forms you bring along two forms of the following identification:

  • One form of photographic ID – for example: Passport, Photocard Driving Licence
  • One proof of address – for example: a recent utility bill with your home address listed

Once these forms have been completed the reception staff will enter your details onto our medical database and make an appointment for a new patient check if required with either:

  • If no regular medication required: the Health Care Assistant/Practice Nurse
  • Regular Medication: an appointment with the Practice Pharmacist or the GP

All new patients will be given a copy of our Practice Leaflet and will be offered the opportunity to be issued with a PIN number that will allow you to register for the online services website and register for text message confirmations and reminders.

This enables you to make and cancel appointments and order repeat prescriptions on-line.

We endeavour to process registrations immediately to enable a smooth transition from one practice to another.  Please note though that it can sometimes take several weeks for records to transfer from your previous GP practice.  This means it is very helpful to bring in any copies of old prescriptions or relevant letters if you require medication during this period.

What if the surgery doesn’t accept me?

Sometimes, you may not be able to register with a GP practice, for example, if:

  • you live outside the area that the practice covers
  • the practice isn’t accepting new patients

You will need to register with another GP practice in your area instead.

It is also important to note that you can only register permanently with a GP practice if you are staying in the area for more than 3 months. If you are not, you may still be treated but as a Temporary Resident.

New Patient Register

To make things easier for you to register, download a Registration Form  and Questionnaire Form.  Complete these at your leisure, print them off and bring them into the practice. It is practice policy to ask for the completed forms to be accompanied by photographic ID and proof of your address. Click here for more information on the Registration process.

The practice has a dedicated Mental Health Triage Nurse working with the practice team  offering patient appointments on a Tuesday each week.
