MEDICINE WASTAGE

Wasted medicines cost NHS Fife £2.1 million last year which could be used to fund other health services in Fife.  If you think you are receiving medicine that you do not need/want, please complete a Medicine Waste Form and return it to the Practice.  Our Pharmacists will check and delete any medication you no longer require.

Thank you.

Urine Samples - Suspected Urinary Tract Infection

If you suspect that you may have a urinary tract infection, you will be required to hand in a urine sample to the Practice.  Due to a recent change by the Laboratories in the way they process urine samples and to assist our nursing team in appropriately requesting the relevant test in order to avoid any samples being rejected, we have introduced a slip which we require to be completed before handing in your urine sample to the Practice.

Please print off the slip below and complete it prior to handing in your urine sample.  Alternatively, complete the relevant slip at the Practice when handing in your sample.

Suspected UTI Slip

Any samples that are handed in to the Practice without the relevant slip will not be tested and will be disposed off.
